Submit Your Physical

• All student-athletes at JWU must have a physical form, filled out in its entirety and signed by a physician, on file in Health Services. The form can be found HERE. It can then be uploaded to Health Services through the pyraMED student patient portal.

 
Complete SportsWare

• All student-athletes at JWU must complete all required information thru SportsWare Online.

Sickle Cell Requirement for NCAA Athletes

• Student-athletes MUST complete one of the options below: 
  • Student-athletes may obtain a sickle cell blood test. 
  • Student-athletes can obtain their newborn sickle cell test results. 
    • Each state has a database. You can find the contact for your state here: State Contact List
    • This directory also provides access to a database to find results: Database 
• Please upload your test results to Sportsware
  • visit Sportsware website
  • Click 'Form' on left side of page
  • Click 'Add'

Athletics Insurance Information

• All athletes must watch the following video containing pertinent information about insurance prior to any athletic participation: VIDEO
• If you have opted out of the insurance through University Health Plans and are using your own personal health insurance to cover you while you compete in athletics, please check with your insurance carrier to be sure that you will be covered for routine appointments and/or emergencies that occur from athletics.  Also, verify that your insurance will cover you while out of state (if applicable).  If you have any questions or concerns, please call 401-598-1626 and we will be happy to help you.

Concussion Information

• All athletes must watch the following video containing pertinent information about concussions prior to any athletic participation: VIDEO
• After completion, each athlete must sign a form acknowledging that they have watched the video and understand its contents. Also included on this video is the NCAA concussion fact sheet that you may also read for additional information. The signed form will be handed in/emailed to your team athletic trainer.  Any questions or concerns can be addressed with a member of the athletic training staff.
