Physical Information

• All club team student-athletes at JWU must have a physical form, filled out in its entirety and signed by a physician, on file in Health Services.  The form can be found HERE.  It can then be uploaded to pyraMED student patient portal.

All Athletes Must Watch Following Informational Videos Annually
