Physical Information

• For first year members of IDA and IHSA: All student-athletes at JWU must have a physical form, filled out in its entirety and signed by a physician. A form can be found HERE. It should then be uploaded to Health Services through the pyraMED student patient portal as well as into Sportsware.

 
Complete SportsWare

• All new Equine student-athletes at JWU must complete all required information thru SportsWare Online
  • Step-by-step instructions may be found HERE
• All returning Equine student-athletes must update all required information through SportsWare Online.  
  • DO NOT CREATE A NEW ACCOUNT. LOGIN TO YOUR CURRENT ACCOUNT WITH YOUR JWU EMAIL ADDRESS.
